Our community outreach team works across Chicago's neighborhoods to connect individuals experiencing homelessness with immediate resources and long-term support. We don't wait for people to come to us — we go to them.

Street Outreach
Daily outreach teams canvass Chicago neighborhoods, building trust with unsheltered individuals and connecting them to housing, healthcare, and services.
Resource Fairs
Monthly community resource fairs bringing together 20+ service providers to offer housing, healthcare, legal aid, employment, and more in one location.
Neighborhood Engagement
We partner with block clubs, faith communities, and neighborhood associations to build community-level support networks.
Case Management
Intensive, individualized case management connecting clients to housing, benefits, healthcare, and employment with ongoing follow-up.
Emergency Resource Distribution
Distribution of food, clothing, hygiene supplies, and emergency supplies to individuals in need across the city.
Community Partnerships
Collaboration with 75+ local organizations, hospitals, schools, and government agencies to create a seamless service network.
